The Charles Scholarship Nomination Form

We welcome nominations from high school counselors and community-based organizations for the Charles Scholarship by December 15, 2026.

As a reminder:

Nominees must attend a Chicago Public School or Chicago-area charter school.
This nomination form will require a letter of recommendation for the nominee.
There is no limit to how many students you can nominate for the Charles Scholarship.
Nominees should apply and submit all admission and financial aid materials by their chosen application deadline.
