Music Scholarship Application Form

To be considered for a Music Scholarship, after completing this scholarship application form you should:

1. Upload your audition video file(s) to your applicant portfolio. You can use the uploader included on your applicant portal (available after you have submitted an application).
2. Alternatively, you may submit a URL that links to your media file. If using a web service that shortens URL addresses, please be aware that it is possible your submission may not be reviewed before the shortened link expires.
