    SCHOOL NOMINATION FORM

    Each high school may nominate one or two students.

    Instructions for Counselors

    • Complete this form for your school's nominee(s). (One student nomination per form.)
    • Attach an official transcript.
    • Attach a letter of recommendation for your nominee.
    • Completed nomination materials must be received by November 1 of the student's senior year.
    • Email confirmation of your nomination will be sent to you and your nominee(s) within a few days after receipt of the nomination materials. 
    • Support your student in completing application to Davidson.  All forms are available online at www.davidson.edu/apply.
    • All admission materials must be received no later than 5:00 p.m.,  November 22.
